A round mass in the left middle fossa, well-defined, with a fat-fluid level inside. There is no edema in the surrounding brain parenchyma.

In the left middle fossa, there is a well-defined, round mass measuring approximately 30 x 32 x 26 mm. The mass contains a fat-fluid level and shows no diffusion restriction or enhancement. It compresses the temporal brain parenchyma without causing brain edema. Additionally, it displaces the left internal carotid artery - cavernous segment toward the midline.

Histopathology

I. Gross description: the specimen appeared white, grey and yellow, measuring from 2 x 2 x 1 mm to 6 x 5 x 4 mm.

II. Microscopic description: on a slide prepared from the received specimen and stained with Hematoxylin and Eosin (HE): a cyst covered with stratified squamous epithelium, containing numerous keratin layers. The cyst wall has benign morphology, closely arranged sebaceous glands, and outside the cyst wall, there is thick fibrous tissue. No malignant features are observed.

Conclusion: Dermoid cyst.

Based on the location and findings on CT, MRI, the most likely diagnosis is dermoid cyst.

The patient underwent surgery to remove the lesion and the pathology confirms dermoid cyst.
