Presentation

Left supraorbital swelling.

Patient Data

Age: 4 years
Gender: Male

Large well-circumscribed expansile and lobulated intradiploic cystic lesion of left supraorbital location measuring 42 x 38 x 28 mm. It displays a marked high signal on T1 and FLAIR, high signal on T2 with no restricted diffusion or enhancement on post-contrast sequences. 

Case Discussion

MRI features of an intradiploic expansile cystic lesion of high signal on T1 with no restricted diffusion suggestive of a white epidermoid cyst.

The high signal on T1 sequences is due to the high protein or keratin content with variable signal on T2 sequences (hypo or hypersignal) and restricted diffusion 1,2.
