Presentation
Numbness of the lower limbs.
Patient Data
Soft tissue mass of fatty density of the spinal canal probably of intradural location with intramedullary component expanding the spinal cord. No calcification or bony remodeling is seen.
Well-circumscribed lobulated intradural mass with intramedullary component extending from D3 to D6 level with no extension through the neural exit foraminae. It elicits a high signal on T1 and T2, attenuated on fat sat sequences with no enhancement on post-contrast sequences. No calcification or hemorrhagic component is seen on the GE sequence. No evidence of bony remodeling.
Case Discussion
CT and MRI features most consistent with an intradural spinal lipoma with intramedullary component.
On imaging, the main differential diagnosis includes:
-
mixed-signal intensity
may be associated with a dermal sinus
-
is a form of occult spinal dysraphism
mass is often palpable
may have cutaneous stigmata
