This lesion was described as an intramammary lymph node. This one has a fatty hilum.

Case Discussion

The position and appearance are typical of an intramammary lymph node. They are mostly seen in the upper outer quadrant of the breast and typically are well-defined, smoothly circumscribed lesions of low density with a hilum or a fatty deposit. These nodes are usually stable in position and size with follow-up. 

If the lesion is dense and spiculated it is not a lymph node.
