Case contributed by Ammar Haouimi
Diagnosis probable

Presentation

Progressive painless swelling of the dorsal aspect of the left elbow.

Patient Data

Age: 30 years
Gender: Male

The ultrasound images show a vascularized fusiform heterogeneous hypoechoic soft tissue mass within the anconeus muscle, containing a phlebolith with preserved muscle architecture.

There is an ill-defined soft tissue mass of tubular pattern within the anconeus muscle, iso-to mildly hyperintense on T1WI, markedly hyperintense on T2WI and T2WI fat sat, containing small channels as well as a punctate phlebolith of low signal intensity. The postcontrast sequences show a progressive and heterogeneous enhancement. Note a bone remodeling of the adjacent cortical ulna.

Case Discussion

MRI appearances highly suggestive of an intramuscular hemangioma within the anconeus muscle
