Presentation

Right thigh swelling for 2 months, sudden increase in size last week

Patient Data

Age: 25 years
Gender: Male

About 11 x 8 x 8 cm heterogeneous complex cystic lesion seen within the anterior-medial intramuscular compartment of upper right thigh with thickened edematous adjacent subcutaneous fat containing multiple small daughters’ cyst and detached hypointense thin membrane post-contrast shows marginal wall enhancement

The heterogeneous lesion is seen within the anterior-medial intramuscular compartment of the right upper thigh with thickened edematous adjacent subcutaneous fat containing multiple small daughters’ cysts with echogenic wall and detached echogenic floating membrane with no significant vascularity in color doppler, few reactive inguinal lymph nodes.

Case Discussion

The presence of echogenic floating membrane and daughter cysts going with hydatid cyst.

History of a sudden increase in the size of the lesion and feeling of pain in addition to perifocal thickened adjacent fat and edema is seen mainly in fat suppression sequence in addition to the heterogeneous enhancement and reactive inguinal lymph nodes indicate an additional infectious process.

Intramuscular hydatid infection was suspected.
