Intraperitoneal traumatic bladder rupture

Presentation

Admit to emergency department with symptoms of urinary retention. Placement of urinary cathether in ED.

Patient Data

Age: 60 years
Gender: Male

The urinary catheter balloon is located outside the urinary bladder.

Intraperitoneal free fluid of density of 15 HU, probably urine, is noted in the pelvis and right paracolic gutter.

On the excretory phase, there is no active leakage of contrast outside the bladder.

In addition, significant diffuse hepatic steatosis is seen.

Case Discussion

The patient admitted he had suffered bicycle trauma a day before admission, with injury by the bicycle handlebar. After placement of the urinary bladder catheter, blood was seen in the urine. After this CT examination, the patient was transported to the operating theater and bladder dome rupture was confirmed and successfully repaired. In the examination, no active leakage of contrast from the bladder is noted. We believe that the catheter balloon could have blocked defect in the bladder wall.
