Intratendinous ganglion - wrist

Presentation

Right wrist dorsal nodular swelling with mild pain for the last few months. No trauma.

Patient Data

Age: 45 years
Gender: Female

The clinically visible nodule is a well-defined lesion involving the extensor tendon of the ring finger. The dynamic scan showed nodule movement along with ring finger movement. It measures about 10 x 7 x 6 mm. The lesion is anechoic with a few tiny internal echoes and a thin septa. Acoustic enhancement is present. There is no calcification or vascularity. There is tendon sheath mild effusion involving ring finger extensor tendon. The tendon shows continuity.

The rest of the extensor compartments were normal without tenosynovitis. There is no joint effusion.

Case Discussion

A female presented with wrist dorsal nodular swelling with mild pain. On examination, the nodule showed movement along with finger movements. The ultrasound shows a ganglion cyst involving the ring finger extensor digitorum tendon along with mild tenosynovitis.

Ganglia are the most common masses involving wrist and hand 1,2. The majority of the wrist ganglion cysts are located on the dorsal side which originates from the scapholunate joint or ligament 2. Ganglion cyst of extensor tendons is rare 1,3.
