Intermittent left groin pain and relatively small left testis.

Intratesticular varicocele

Left testis is seen relatively small for patients age averaging 5 cc in volume. Prominent tortuous tubular structures extending from the testicular mediastinum towards the periphery showing reflux during Valsalva maneuver. Extratesticular varicocele were also present.
Case Discussion
Although intratesticular varicoceles are rare they can be seen in clinical practice. Intratesticular varicoceles are more common on the left side. They should be differentiated from ectasia of the rete testis and intratesticular cysts, this could be done by demonstrating color flow on Valsalva maneuver using color Doppler.
