Intrauterine testicular torsion

Presentation

Born several hours ago. Hard mass palpated in right scrotum, blackened scrotal skin. Left testis palpated in the canal.

Patient Data

Age: 3 hours
Gender: Male
ultrasound

The scrotal skin is thickened.
Enlarged heterogeneous right testis measuring 1.6 x 1.7 cm without demonstrable flow. ​Minute amount of surrounding fluid. Likely testicular torsion.
Left testis in lower abdomen.

Case Discussion

From the surgical report:

Right testis is black, necrotic, cloaked in black necrotic tissue. The cord is torsed 360 degrees. After performing detorsion, dousing with lukewarm water, and waiting for 15 minutes, the testis did not recover. Gradual incisions into testicular tissue, attempting to arrive at salvageable tissue, but entire testicular tissue was black, wherefore orchiectomy was decided on.
An attempt was made to palpate and fix the left testis but it could not be palpated in the scrotum or the canal.

Pathology report:

Macroscopic description: necrotic testis wrapped in pliable brown tissue.
Microscopic description: diagnosis: necrotic and hemorrhagic testis.

Since there was no calcification in the testis, it can be assumed that the torsion had been recent, i.e. past few days, and therefore antenatal.
