Patient Data

Age: 60 years
Gender: Female

Screening mammogram

12 mm irregular mass in the lower, inner quadrant of the left breast.

Workup mammography

12 mm mass of soft tissue density with spiculated margins.

Diagnostic ultrasound

10 mm hypoechoic mass in the 7 o'clock position of the left breast. Size and position corresponds to the mammographic mass. Screening ultrasound of remainder of left breast and axilla reveal no suspicious features.

Case Discussion

Left wide local excision: 13 mm, grade 3 invasive carcinoma NST. No lymphovascular or perineural invasion. Clear of margins. Negative single sentinel lymph node. Note marked hypoechoic ultrasound appearance with posterior acoustic enhancement. This should not be confused with a cyst. It often reflects the high grade/cellular nature of the high grade carcinoma.
