Invasive mucormycosis of the maxillary sinus

Presentation

Double vision, photophobia, pain on moving eye, right maxillary pain, numbness and swelling. FNE: pus and crusiting right nasal cavity with edema of maxillary meatus.

Patient Data

Age: 60 years
Gender: Female

Right maxillary sinusitis with extension into the orbit through the orbital floor. Inflamed inferior rectus muscle and right sided proptosis. Inflammatory change tracks posteriorly into the pterygopalatine fossa.

1. Right maxillary sinusitis with extension into the orbit through the orbital floor. 

2. Maxillectomy, anterior and posterior ethmoidectomy, right sphenopalatine artery ligation and medial orbital wall decompression. 

3. Right orbit exenteration. Disease involves right pterygopalatine fossa, orbital apex and cavernous sinus. 

Case Discussion

Case highlights the disasterous impact of invasive fungal sinusitis. Treatment is often very aggressive with poor outcomes.
