IPMN and pancreatic adenocarcinoma

Presentation

Painless obstructive jaundice.

Patient Data

Age: 80
Gender: Male
Fluoroscopy

Case Discussion

FINDINGS:

ERCP
Irregular stricture distal CBD.
Filling defects pancreatic duct.

CT
Dilated intra and extra hepatic bile ducts.
Dilated pancreatic duct with no mass seen.
Small foci of calcification in the tail of the pancreas.
Gallstones.
No abnormal lymph nodes or metastases seen.

LKELY DIAGNOSIS:
IMPN - main duct type.
Malignant change, causing biliary obstruction.

DIFFERENTIAL:
Mucinous pancreatic neoplasm.

FURTHER INVESTIGATION/MANAGEMENT:
Palliative stenting
