Isolated left-sided pulmonary artery agenesis with left lung hypoplasia

Presentation

Respiratory distress. Chest x-ray (not shown) shows opaque left hemithorax.

Patient Data

Age: 4 months
Gender: Male

Absence of the left pulmonary artery.

Decrease volume of the left lung with mild mediastinum deviation to the left side.

Normal main pulmonary artery and right pulmonary artery.

Normal trachea and both main bronchi.

Case Discussion

Unilateral absence of pulmonary artery is a rare anomaly and usually presents in children and adults with respiratory distress. It may be associated with cardiovascular anomalies like coarctation of the aorta, VSD, ASD, and tetralogy of Fallot. Chest x-ray usually demonstrates volume loss of the affected side with an ipsilateral mediastinal shift, but the diagnosis is established by imaging methods like CT angiography and MRI.
