Presentation

Trauma. Unable to weight bear.

Patient Data

Age: 20 years
Gender: Male

Ankle joint effusion seen as an opacity protruding anteriorly from the ankle joint on the lateral projection. An undisplaced posterior malleolus fracture is also seen on the lateral projection. 

Case Discussion

Isolated posterior malleolus fractures are rare. They are the result of a combined axial loading and compression mechanism, and were initially described as "paratrooper fractures" due to their known association with parachuting. The mechanism in this patient is unknown.
