IUCD related uterine perforation -silent

Diagnosis almost certain

Presentation

History of fall on the ground 3 days back. Complaining of backache.

Patient Data

Age: 45 years
Gender: Female

There is no vertebral compression fracture. Disc spaces are maintained. There is no lysis or listhesis.

There is an 'intra-uterine' device in the anterior side of the left lower abdomen with horizontal orientation.

There is no abnormal bowel gas pattern.

US was done to localized IUD

There is a linear structure (with two parallel lines and bifurcation at one end ) in the left lower abdomen. It casts an acoustic shadow. It is just deep to the left rectus and lateral to inferior epigastric vessels. It is surrounded by fat. It moves with respiration. The uterine cavity is empty.

Case Discussion

According to the Patient, IUCD dislodged about 21 years back. Two-time ultrasound was done and the uterine cavity was reported as empty. It was presumed the IUCD was outside the body. A radiograph was not done.

Findings suggest an intraperitoneal IUCD with silent uterine perforation.
