Presentation
Left flank pain and fevers. ? Pyelonephritis.
Patient Data



Unfortunately, movement artifact degrades some of the images, particularly in the regions of the kidneys. Allowing for this, there is some mild left perinephric fat stranding. No perinephric fluid collection. No abnormal renal perfusion. No dilatation of the renal collecting systems. The urinary bladder is normal.
The liver, pancreas, spleen and adrenal glands are normal. The gallbladder has been removed.
No free fluid or free gas. Bowel is unremarkable. No lymphadenopathy.
A rectangular metallic clip is projected around the infrarenal IVC, significantly narrowing the vessel lumen. Several enlarged lumbar veins bypass the narrowing.
IMPRESSION
Mild left perirenal fat stranding could be secondary to a lowgrade nephritis. No evidence of perinephric collection or renal tract obstruction.
IVC clip, please correlate with surgical history.
Case Discussion
The treating team where able to obtain a further history of a pregnancy-related DVT several decades ago which was treated with open thrombectomy and clipping of the IVC.
