IVC leiomyosarcoma

Presentation

Vague abdominal discomfort.

Patient Data

Age: 55 years
Gender: Male

Large, lobulated soft tissue mass along the posterior infrahepatic IVC invading into the IVC lumen. Adjacent right adrenal gland but the mass does not convincingly appear to arise from it. Slight contact of the posterior margin of the mass with the right diaphragmatic crus. 

Case Discussion

Biopsy and surgically proven IVC leiomyosarcoma. The mass is centered on the IVC and invades into the lumen, but does not invade into other structures, helping to support a primary vascular tumor (most commonly leiomyosarcoma). Another important consideration is an adrenocorticocarcinoma, which can also invade into the IVC, but in this case, does not appear to be convincingly involved.
