Chronic cough.

Age: 70-year-old
Gender: Male

Chest Radiograph

No evidence of pulmonary consolidation to suggest infection. Pleural spaces are clear. The tubular structure along the right mediastinal contour with an air-fluid level within is likely a pulled up stomach (Ivor Lewis procedure).

No features to suggest lung infection. Gastric pull-up procedure with an air-fluid/contrast level within. 

Case Discussion

Classic case of Ivor Lewis procedure (also known as a gastric-pull up), a type of oesophagectomy usually performed in patients with oesophageal cancer.
