Diffuse and symmetrical cerebellar atrophy.
Bilateral swelling and T2-FLAIR hyperintensity of cerebellar peduncles that extends towards cerebellar hemispheres.
Cruciform T2-FLAIR signal hyperintensity of the pons (hot cross bun sign).
Case Discussion
JC virus PCR assay was positive.
The case is consistent with JC virus granule cell neuronopathy.
