Superior cerebellar peduncles are thickened elongated with midline cleft at the pontomesencephalic junction, giving molar tooth appearance. Hypoplasia of inferior vermis giving bat wing appearance of fourth ventricle.

MRI appearances are typical of Joubert syndrome. Note the molar tooth sign.
