Hypoplasia of the cerebellar vermis with prominent thickened elongated superior cerebellar peduncles gives the midbrain a characteristic molar-tooth sign appearance with a bat-wing configuration of the fourth ventricle.

The MRI findings are characteristic of Joubert syndrome.
