Presentation

Recurrent facial pain for months.

Patient Data

Age: 10 years
Gender: Male

Mildly enlarged bilateral parotid glands with heterogenous echotexture and multiple smaller hypoechoic areas within (right worse than left).

No calculi. No dilated ducts. No mass. No lymphadenopathy.

Case Discussion

Juvenile recurrent parotitis is the second most common cause of parotitis in childhood (mumps infection being the first). Clinically it presents with multiple episodes of facial pain +- fever/malaise. It can mimic mumps clinically.

Pathology is unknown - commonly bilateral changes on imaging but symptoms may be unilateral. Sialography is similar to Sjögren syndrome.

Prognosis is good - most self-resolve with time (2nd decade of life). Treatment is supportive.
