Presentation

Chronic right wrist pain

Patient Data

Age: 60 years
Gender: Female
mri

Diffusely altered marrow signal of the lunate bone is noted in the form of T1 hypointense signal and T2 hyperintense signal with subchondral cystic changes along its proximal part.

x-ray

Mildly increased density, cortical irregularity, and cystic changes of the lunate bone.

Case Discussion

The MRI signal changes within the lunate reflects sclerosis which is confirmed with the plain X-ray, along with the cystic changes and cortical irregularities, are most consistent with avascular necrosis of the lunate (Kienbock's disease). There is no negative ulnar variance, which is a common association.
