Both carotid arteries assume a retro-pharyngeal course (kissing carotids).
Case Discussion
The origin of the ICA normally is posterolaterally in relation to the ECA. In 10% of cases, it may originate medially and both carotids approximate each other in the midline giving what is called kissing carotids.The importance of kissing carotids is: may mimic intra-sellar pathology- catastrophic if unreported before retropharyngeal surgery
