Anterior knee pain; aggravated by playing and running. No history of trauma.

Irregularity and sclerosis of the primary ossification center of the patella.
Case Discussion
Osteochondrosis of the primary ossification center of the patella (Köhler's disease of the patella) has been reported as a rare cause of anterior knee pain in children between 5 and 9 years of age. The etiology remains unclear. It affects the single knee usually but bilateral cases have been reported in the literature.
This entity should be distinguished from Sinding-Larsen-Johansson’s disease, which is an osteochondrosis of the secondary ossification center of the patella usually affecting the lower pole.
