L3/4 disc migration

Presentation

Long history of left sided back pain, but new severe pain on right, radiating down right thigh. Less than one week of symptoms. No bowel/bladder disturbance.

Patient Data

Age: 60 years
Gender: Male

L3/4: Subarticular and foraminal disc extrusion with foraminal migration, extending cranially and filling the right L3 exit foramen. The right L3 nerve is certainly contacted in this scanned position and likely compressed. Centrally, there is a minor disc bulge and facet / flaval degeneration with mild bilateral recess stenosis but no cauda equina compression. Mild left L3 exit foraminal stenosis.

L4/5: Fatty degenerative endplate changes. Mild disc height loss, minor disc bulge and moderate facet / flaval degeneration resulting in mild canal narrowing and moderate bilateral recess and L4 exit foraminal stenosis. Fat remains visible around the exiting nerves.

Normal vertebral body height and alignment. Normal included distal cord, with normal signal. Conus at L1 level. No concerning extraspinal finding.
