A large well-defined oblong fluid-filled cystic structure in the right iliac fossa extending to the pelvic region. It shows a thin regular wall contiguous to the base of the cecum with a homogeneous fluid content of a low signal on T1, and a high signal on T2 with no restricted diffusion. Thin peripheral enhancement on postcontrast sequences. No solid component or mural calcification is seen. No surrounding fat stranding or intraperitoneal effusion. No thickening of the peritoneal reflections.

Case Discussion

The CT features are most consistent with a large appendiceal mucocele.

Appendiceal mucocele occurs when obstruction of the appendiceal lumen causes accumulation of mucus with progressive distension of the appendix. The underlying causes lie on a spectrum between benign obstruction with retention cysts and malignant obstruction due to invasive mucinous adenocarcinoma.
