Large intradiploic epidermoid cyst

Presentation

Progressive painless frontal swelling over 25 years.

Patient Data

Age: 45 years
Gender: Male
ultrasound

There is a large cystic mass of homogeneous echogenic content with posterior acoustic enhancement of left frontal location with intra-and extracranial components. No calcification is seen.

There is a large well-defined intradiploic cystic mass arising from the left frontal region with homogeneous content isodense to the cortical grey matter with no enhancement following IV contrast. It shows complete destruction of both inner an outer tables of the skull with extension to the left frontal sinus. There is a mass effect on the on the left frontal lobe, frontal horns and midline structures with subfalcine herniation.

Case Discussion

The ultrasound and CT features are suggestive of an intradiploic epidermoid cyst with intra-and extracranial extension.
