Diagnosis certain

Presentation

Ultrasound performed for right upper quadrant pain regarding clinical presentation with presumed cholecystitis. Incidental renal abnormality identified.

Patient Data

Age: 55
Gender: Female
ultrasound

Large hyperechoeic mass in the right kidney, in keeping with a AML.

The mass is well circumscribed, without any significant Doppler activity within.

mri

Large mass in the upper and mid-pole of the right kidney insinuating into the renal sinus fat.

The mass is well circumscribed and follows fat across all sequences.

Findings in keeping with a angiomyolipoma (AML).

Fluoroscopy

Static image from coil-embolization procedure.

Follow-up imaging post procedure is scheduled.

Case Discussion

The kidneys are a common site for angiomyolipomas, and are often incidental findings.

This case beautifully demonstrates fat suppression on MRI - the lesion is almost entirely fat, although content can be variable.

Larger lesions may hemorrhage and hence require treatment if larger than 4cm, typically with embolization.
