Late hyperacute ischemic stroke

Presentation

Patient presented with abrupt onset speech change. She underwent NCCT head followed by MRI brain within 24 hours of symptom.

Patient Data

Age: 55 years
Gender: Female

There is an ill-defined, linear, subtle hypodense area noted in the sub-cortical region of left frontal lobe. 

There are few, tiny, subtle, hyperdensities without surrounding edema noted in the bilateral basal ganglia suggesting calcification.

There are few, subtle, subcentimetric, T2/ FLAIR hyperintense areas which show diffusion restriction on DWI/ ADC noted in the sub-cortical region of left frontal lobe suggesting infarction.

SWI shows few blooming artifacts in the bilateral basal ganglia which appear dark on SWI phase imaging suggesting calcification.

Case Discussion

Given the history of the patient and imaging findings, this case is of late hyperacute ischemic stroke.
