Presentation

Left hemiataxia with vertigo in a hypertensive patient.

Patient Data

Age: 60 years
Gender: Male

Focal area of high T2, and FLAIR with restricted diffusion involving the left lateral medulla oblongata with no hemorrhagic component on GE sequence in keeping with an acute focal infarct.

Cerebral volume loss with small vessel ischemic changes.

Case Discussion

MRI features are most consistent with lateral medullary syndrome (LMS) or Wallenberg syndrome

The Wallenberg syndrome is most commonly due to an occlusion of the intracranial portion of the vertebral artery followed by PICA and its branches. The main risk factors are hypertension, followed by smoking and diabetes mellitus.
