Lateral medullary syndrome (Wallenberg syndrome)

Presentation

Dizziness, disequilibrium, dysphagia.

Patient Data

Age: 60 years
Gender: Male

Wedge-shaped T2 hyperintensity in the right lateral posterior aspect of the medulla. Area is showing diffusion restriction.

Lack of arterial flow in the right vertebral artery, indicative of occlusion.

Scattered bilateral supratentorial T2 hyperintensities.

Case Discussion

The vertebral artery gives rise to the posterior inferior cerebellar artery which then supplies also the ipsilateral lateral and posterior aspect of the medulla.

The symptoms of the lateral medullary syndrome include vestibulocerebellar symptoms, autonomic dysfunction and ipsilateral bulbar muscle weakness.
