Lateral patellar dislocation injury with detached osteochondral fracture

Presentation

Patient came on a wheelchair suffering from right knee joint pain post accidental falling down at home x 1 day.

Patient Data

Age: 16
Gender: Female

Chip avulsion fracture of the patella with joint effusion.

-Absence of articular cartilage at the medial patellar facet, with mild subjacent bone marrow edema and a small (1.4 cm linear ) floating osteochondral fragment within the medial knee gutter.

- Lateral femoral condyle  bone marrow edema " contusion".

- Medial patellar retinaculum injury with lateral patellar displacement.

- Grade I proximal MCL injury.

- Large knee joint effusion is noted with fluid/fluid level (hemarthrosis).

- Muscle edema and contusions in vastus lateralis and medialis.

- Rest of the visualized other extensor mechanism structures are preserved.

- Lateral collateral complex and cruciate ligaments are preserved.

- Menisci are unremarkable.
