Presentation

Smoker with cough. Chest infection or other?

Patient Data

Age: 65 years
Gender: Female
x-ray

Triangular density projected behind the left side of the heart.

The left hilum is inferiorly displaced being level with the right hilum rather than its typical position higher in location relative to the right.

Volume loss in the left hemithorax.

Right lung clear.

Heart size normal.

Case Discussion

Classical appearances of a tight left lower lower lobe collapse.

The triangular or sail shaped density behind the heart indicating a tight collapse.

Secondary signs of an inferiorly (pulled) displaced left hilum and left hemithorax volume loss are also present.

In this case lung cancer is the cause until proven otherwise and indeed this was the case in this patient.
