There is a dense opacity behind the heart, adjacent to the mediastinum. This represents tight collapse of the left lower lobe.
Case Discussion
Features here are of left lower lobe collapse. In a child, the differential should include mucous plug and aspirated foreign body as well as much less common intrinsic and compressive pathology.
