Presentation
Post-menopausal lady with episodic excessive sweating (before sleep) and palpitation for 1 year.
Patient Data
Irregular solid nodule with spiculated margins measuring about 9 x 15 mm seen in the peripheral lateral basal segment of the right lower lobe. No calcification, cavitation or fat component is seen in it. Rest of the lungs are clear. No mediastinal lymphadenopathy, pleural thickening/effusion or pneumothorax is seen. Adrenals are normal. No concerning lytic or sclerotic bony lesions.
Histopathology report of the resected right lower lobe pulmonary nodule.
Case Discussion
Imaging features are suggestive of a malignant pulmonary nodule (likely primary malignancy). Further imaging work up (CT abdomen, mammogram etc) was negative.
The patient underwent resection of this pulmonary nodule via video assisted thoracoscopic surgery (VATS) and the histopathology showed lepidic predominant invasive adenocarcinoma.
