Level 2 endopelvic fascial defect and levator dehiscence

Axial image: marked thinning of the puborectalis muscle on the right side, with loss of insertion on the pubic bone (compared to normal on the left side). Lateral laxity of the vagina on the right side, caused by loss of muscular support and also loss of supporting lateral level 2 endopelvic fascia.
Coronal image: asymmetric appearances of levator muscle, with no normal puborectalis or iliococcygeus fibers seen.
