The left cerebellar hemisphere hypertrophy with the characteristic tigroid appearance with mass-effect on cerebellar tonsils, protruding into the foramen magnum causing mild occlusive hydrocephalus

T2-shine through effect is seen on DWI, there is no diffusion restriction

The right maxillary sinus mucosal thickening
