Presentation
Sudden onset left groin pain whilst running.
Patient Data
Small avulsion fragment in the medial left femoral head adjacent to the fovea, at the presumed attachment of the ligamentum teres.
There is an old ununited avulsion fracture of the left lesser trochanter. The left superolateral acetabular margin is irregular suggesting previous trauma, however, no acute fracture is demonstrated.
CT confirms an acute avulsion fracture of the left femoral head medially at the superoanterior aspect of the fovea capitis where the ligamentum teres inserts. Acute left hip joint effusion noted.
There is an old ununited fracture of the left superoposterior acetabular margin. Old ununited avulsion fracture of the left lesser trochanter also demonstrated.
Case Discussion
Ligamentum teres avulsion fractures are rare; however, rupture and tears of the ligament are an underdiagnosed injury, often only found at arthroscopy.
