Lipoma of the filum terminale

Note the fatty density in the middle of the thecal sac reaching down to L2. 

Case Discussion

A fatty flium is not an uncommon incidental finding. Care should be taken in assessing for the position of the conus (not possible on these images) to determine if it is low lying implying a tethered cord.
