Presentation
The patient presented for evaluation of an enlarging mass within the left forearm.
Patient Data
There is a soft tissue mass dorsal and lateral to the distal third of the radius, which demonstrates a mix of water and fat density components, making a simple lipoma unlikely.
There is also enthesopathy at the olecrenon process, cortical irregularity and thickening of the proximal radius and ulna along the interosseous membrane, and mild subchondral cystic change of the proximal lunate.
There is a predominately high T1 low STIR signal mass along the radial aspect of the distal forearm which appears to arise from the abductor pollicis longus; however, the mass is heterogenous with multiple thickened low T1, high STIR signal components which avidly enhance.
Case Discussion
The thickened, nodular, enhancing septa within a predominantly fatty mass are typical of a liposarcoma.
