The observation in the anterior segment right lobe measuring 32 x 31mm presents with non-rim hyperenhancement on the arterial phase, with washout on delayed phase.

Case Discussion

The observation has features: lesion >20mm, arterial phase non-rim hyperenhancement, and washout on a later phase, which classifies the lesion as LI-RADS 5 (definitely HCC). Interestingly, there is no washout on the portal venous phase, but the washout on the delayed phase on CT still qualifies, since there is a nonperipheral visually assessed temporal reduction in enhancement in whole or in part relative to composite liver tissue from earlier to later phase.
