Diagnosis almost certain


Deteriorating renal function. Background history of depression treated with lithium.

Patient Data

Age: 60 years
Gender: Male

Representative images from renal ultrasound demonstrating bilateral poor corticomedullary differentiation. 

Additionally there are numerous punctate echogenic foci seen throughout the renal parenchyma bilaterally, likely representing minute cysts.

No hydronephrosis or suspicious mass lesion.

Case Discussion

Lithium-induced renal disease has a unique imaging appearance of numerous uniform microcysts and punctate echogenic foci which may be cortical or medullary in origin.
