Presentation

AKI with likely diabetes insipidus. ?structural cause of renal failure.

Reduction in the cortical medullary differentiation in both kidneys however normal renal lengths bilaterally.

Apparent multiple echogenic foci in both kidneys throughout the cortex and possibly the medulla. No hydronephrosis.

Simple renal cysts demonstrated in the upper pole of right kidney, largest measures 2.2 cm.

Case Discussion

Following review of the patient's clinical record, it was discovered that the patient was on long-term lithium therapy for a psychiatric condition. Given this, it was concluded that the appearances were most likely to represent lithium-induced renal disease.

The lithium therapy was changed to an alternative medication and the renal function subsequently improved.
