Liver haemangioma (mimicking haematoma in the setting of trauma)

Case contributed by Dr Paul Leong

Presentation

Trauma. Car versus tram.

Patient Data

Age: 75 years
Gender: Male

Low density lesion in the left lobe of the liver lateral segments (segments II and III), measuring 6 x 3 x 5cm. There is peripheral nodular enhancement on the arterial phase with centripetal filling in on the portal venous phase study. No evidence of adjacent fat stranding or free fluid. Findings would be consistent with an incidental hepatic haemangioma. The typical imaging features fairly exclude haematoma in the clinical settings of trauma.

Other findings:

  • multiple fractures involving the right medial clavicle, bilateral ribs and L1 vertebral body
  • small bilateral pleural effusion
