Lobar hemorrhage from metastasis

Presentation

Sudden collapse and visual field loss.

Patient Data

Age: 65 years
Gender: Male

A heterogeneously enhancing intra-axial mass is identified in the left occipital lobe measuring 5 x 4 x 5 cm and contiguous with the hematoma. No other enhancing lesions identified. The imaged calvarium and paranasal sinuses are clear.

Large left parieto-occipital hematoma is again demonstrated measuring 5 x 4.5 x 4 cm (AP x ML x CC; approximately 45 cc), extending into the left lateral ventricle. The degree of midline shift (9 mm to the right), left lateral ventricle effacement, surrounding reactive edema and ipsilateral sulcal effacement is similar to the previous scan. Effacement of the right ambient and quadrigeminal cisterns suggests uncal herniation, although this has not changed in appearance compared to the earlier imaging. 

Case Discussion

This patient has known non-small cell lung cancer with metastases.
