Diagnosis probable

Presentation

Fever and right upper abdominal pain.

Patient Data

Age: 12 years
Gender: Male

Right and left kidney

ultrasound

There is a well-defined wedge-shaped hyperechoic area seen in the upper pole of the right kidney which appears devoid of vascularity, with focal loss of corticomedullary differentiation.

The remaining renal parenchyma appears of normal echogenicity and uniform vascularity.

The left kidney appears unremarkable, with obvious echogenicity and vascularity differences between the infarcted portion of the right kidney and the left kidney.

Case Discussion

Initial laboratory tests in this patient revealed leukocytosis with left shift, high CRP, and urine analysis reveals signs of infection.

Another alternative differential diagnosis would be renal infarction.
