Lobar pneumonia

Diagnosis almost certain

Presentation

Cough, high fever, and chills. Also, left side pleurisy.

Patient Data

Age: 65 years
Gender: Male

  • there is left upper and middle lung zones opacity, more homogeneous at the middle zone, which contains few air bronchograms. The lung volume is preserved
  • the left cardiac border is ill-defined/loss of silhouette
  • the left costophrenic angle is blunted indicating effusion
  • the right lung is clear

  • there is left upper lung lobe consolidation of heterogenous density, with multiple air bronchograms and bronchiolograms, and small left pleural effusion
  • lungs volume is normal
  • mild centrilobular emphysema

Case Discussion

This is a case of left upper lobar pneumonia with parapneumonic effusion due to Streptococcus pneumoniae; the most common culprit.
