Localized tenosynovial giant cell tumor

Diagnosis probable

Presentation

Painless soft tissue mass of the left thumb.

Patient Data

Age: 30 years
Gender: Female

There is focal, soft tissue swelling of the left thumb, specifically medial soft tissues, proximal phalanx. There is smooth, extrinsic, pressure erosion of the proximal phalanx, medial cortex.

ultrasound

There is a solid, hypoechoic, non-compressible mass, surrounding the flexor tendon of the left thumb at the level of the proximal phalanx. There is no calcification and no abnormal Doppler flow. Dynamic assessment well demonstrates the smooth and free movement of the flexor tendon within the circumscribed lesion.

Case Discussion

Features suggestive of a localized tenosynovial giant cell tumor. There is the typical volar surface, periarticular, digital (thumb) involvement with an untethered and freely moving flexor tendon.
